Every turn keeps its own append-only ledger. Open any answer and use its Audit tab to see the exact event sequence behind it, including what was refused.

A cross-conversation audit view lands with persistence. The per-turn ledger is the source of truth today — the log has to show what did NOT run, so refusals and held actions are recorded as events rather than omissions.
